Summary
Bridging the Gap is a Hospital-Based Violence Intervention Program (HVIP) which is a multidisciplinary program that brings together hospital staff with trusted community-based partners to provide safety planning, wrap-around services, and trauma-informed care to violently injured people. This position will provide intensive case management services for this intervention program, dedicated to reducing re-injury and mortality rates among youth and adults by providing intentionally injured patients with support during their hospital stay and beyond.

Required Education and Experience
Ability to understand and work with underserved communities, at-risk youth and families. Knowledge of child/adolescent services system. Knowledge of positive youth development pathways. The ability to provide assessment, brief counseling, education/support to youth and adults. Ability to work collaboratively with families and community agencies. Successful experience working with young people in an organized programmatic setting. Demonstrated proactive problem solving, teamwork, innovative thinking, creativity, patience and flexibility. Demonstrated commitment and ability to support and mentor young people in a positive manner. Must agree to a CPS and criminal background check. Must have reliable transportation. Availability to work some evening and weekend hours. Demonstrated experience working in and fostering a diverse environment for faculty, staff, and students or a commitment to do so at VCU.

Preferred Education and Experience
Bachelor's Degree in Human Services field. Case management experience required. Previous employment or volunteer work in human services, community agencies or related non-profit or social change organization a plus; experience with probation/parole or criminal justice system preferred. Hospital and community experience preferred.
