General Description

The HIV program Nurse Care Manager is a member of a care team that provides high quality, holistic healthcare and medical case management to patients living with HIV/AIDS through a client-centered approach using the Chronic Care Model as a framework.  Qualified candidates will have a current RN license. Effective organization, efficient time management skills, and the ability to successfully multi-task are essential to success in this position. Demonstrates PHC iCare values in daily work.

Characteristic Duties

  1. Supports and empowers clients throughout health care continuum to include holistic counseling, problem solving “barriers to care” issues and coaching clients as needed re: treatment and medication adherence, healthy life choices, and self-help behaviors.
  2. Performs nursing assessment/medical triage of acute medical issues and medication side effects and refers or monitors appropriately.
  3. Thoroughly and accurately documents all patient encounters in the electronic medical record and assists with data collection, data management and data integrity according to grant guidelines.
  4. Provides HIV education to clients/families/partners, internal staff and external agencies in an organized and appropriate manner.
  5. Assists clients in accessing support services by serving as a liaison with other community agencies.  
  6. Manages client referrals and follow-up for specialty medical, mental health, substance abuse and community services.
  7. Assists with quarterly program newsletter content and facilitates development and ongoing activities for consumer involvement, as detailed by RWCA grant. 
  8. Participates in meetings and committees within PHC and in the community as requested. 
  9. Assists with yearly patient survey to assess client satisfaction with program services.
  10. Participates in local and national training and education to foster continued professional growth and assure high quality care delivery.   
  11. Provides assistance with direct nursing interventions such as venipuncture, wound care, vaccinations and other identified nursing responsibilities and performs additional duties as assigned.
  12. Ensures compliance with PHC policies and procedures, consistent with accreditation, regulatory, federal and state rules and regulations. 
  13. Maintains patient confidentiality 100% of the time.
  14. Demonstrates PHC iCare values and service excellence in daily work.
  15. Attends PHC meetings as requested. 
  16. Performs other duties as assigned.

 

Minimum Qualifications

  1. Registered Nurse with license recognized by the State of Iowa Board of Nursing and BLS certified.
  2. Demonstrated verbal and written communication skills.
  3. Ability to work collaboratively with a wide variety of people, providers and agencies.
  4. Self-motivated, willing to take initiative and work autonomously.
  5. Willingness to listen, give constructive feedback and take direction from Clinical Program Director and Program Manager. 
  6. Ability to apply critical thinking and assessment skills within nursing framework.
  7. Strong organizational and efficient time management skills
  8. Basic knowledge of Human Immunodeficiency Virus infection and Acquired Immunodeficiency Syndrome.
  9. Computer skills to include Microsoft office. 
  10. Ability to consistently meet department work schedule as designated.

 

Preferred Qualifications

  1. Experience in HIV/AIDS related care 
  2. Experience in community health or public health programs.
  3. Experience in case management and the Chronic Care Model.
  4. Knowledge of community resources 
  5. Advanced computer skills; database experience and desktop publishing 
  6. Baccalaureate degree
  7. Verbal and written language proficiency in English and Spanish.
  8. Experience in a community health center.

 

Typical Work Hours

This position typically works 40 hours per week.

Compensation is dependent on education and experience.
