Gateway Health based in Danville, VA is a healthcare management company. Due to substantial growth, our company is currently recruiting for talented, dynamic and motivated individuals. We are seeking a Nurse Manager to work primarily with Medicare members and populations with Chronic Conditions:

• Must have a current Virginia Board of Nursing or Compact State nursing license
• Must have Strong Verbal / Written Communication Skills
• Ability to Identify gaps in care to reduce the risk of hospitalization and readmissions
• Must possess a broad clinical knowledge base
• Must have strong people skills to establish collaborative relationships with members, families, clinical staff and providers
• Strong organizational, problem-solving and decision-making skills based on clinical judgement and evidence-based guidelines required.
• Ability to work independently within a team environment
• On-call rotation and overnight travel required
• Care Management experience preferred
• Microsoft Office experience and excellent computer skills required
