Gateway Health is seeking professional team player to serve as a Community Health Worker (CHW) to service the Twin County – Galax, VA and surrounding areas. This position will focus and work closely with medical providers, primary care teams and other agencies to improve patient care, outcomes, and connect patients to a primary care provider. The CHW will primarily be working out in the community with specific targeted populations, providing social support and informal counseling, advocating for individuals' and community health needs and providing basic health screenings. Basic data analytics, preparing reports, provider education, and related tasks. Organization and time management proficiencies are a must. Competitive benefits and compensation package offered.
REQUIREMENTS
Candidate must be friendly, flexible, proficient in written and verbal communication, professional appearance and demeanor, working experience in similar roles, and proficient with Microsoft Office Suite. Experience in a healthcare setting is preferred but not required.
PREFERRED EDUCATION/QUALIFICATIONS
*High school diploma or its equivalent
*Certified Community Health Worker preferred
*Ability to work collaboratively and effectively within a team
*Verifiable good driving record and reliable transportation required
