Case Manager Housing Based

As a Housing-Based Case Manager, you will have the ability to truly impact lives by helping people diagnosed with mental illness to remain safe and stable in their own private homes. Our Permanent Supportive Housing program helps individuals to live successfully in the community. We have funding to help with rent and furnishings for those who qualify, and we work to make sure they have comprehensive wrap-around services to help them to be successful.

The schedule for this position is Monday-Friday 8:30am-5:00pm.
Minimum Qualifications:
- Education: Minimum of a high school diploma. An Associate's or Bachelor's degree in a human services field preferred.
- Experience: Entry level. One year of clinical experience working in a human services field preferred. Experience working with individuals diagnosed with a Serious Mental Illness (SMI) or the homeless population preferred.
- Safe Driving Record: Safe driving record required (no more than two moving violations in the last three years, no criminal violations in the last five years).
- Background Check: Candidates will be required to submit to a fingerprint criminal background check and drug screen.
