Home Health Aide - HHA GENERAL STATEMENT OF DUTIES: Home Health Aide shall mean a person who, under supervision of a Registered Professional Nurse has the responsibility to assist individuals with health care, personal care, and home management activities. The HHA must be able to provide covered HHA services according to the client’s Care Plan. DIRECTLY RESPONSIBLE TO: Supervising Nurse CORPORATE COMPLIANCE: Abides by all Corporate Compliance policies and procedures including HIPAA, and maintains confidentiality of all client, family, and employee related information. SERVICES /JOB FUNCTIONS: Home Health Aides provide assistance to disabled, chronically ill and cognitively challenged clients with personal care and health needs in the home setting. Home Health Aide Services may include but are not limited to: a. Report client changes to the Supervising Nurse. b. Making and changing beds. c. Performing home management tasks, including housekeeping, laundry, shopping and errands d. Bathing of the client in the bed, the tub, or the shower e. Dressing f. Grooming, including care of hair, shaving and ordinary care of nails, teeth, and mouth g. Toileting – this may include assisting the client on and off the bedpan, commode or toilet h. Walking, assist with durable medical equipment within the home and outside the home. i. Transferring from bed to chair or wheelchair. j. Preparing and serving simple modified diets as indicated on the Care Plan k. Feeding. l. Collection of routine specimens m. Caring for male condom catheter; assisting with emptying an indwelling catheter collection bag n. Assisting with changing an ostomy appliance/dressing when the ostomy is mature and stable. o. Assisting the self-directed client with the use of oxygen equipment p. Measuring and recording vital signs, including weight q. Assisting with self-administration of medications. r. Provision of routine skin care. s. Performing other related tasks as necessary and that are within the Certified HHA Scope of Functions and Tasks. CHARACTERISTICS: The applicants should: • Be mature, emotionally and mentally stable. A minimum age of 18. • Have experience in home health care, personal care or homemaking is preferred. • Be able to read and write English, understand and carry out directions and instructions, record messages and keep simple records in English. • Have a sympathetic attitude toward providing services to clients at home. • Take a genuine interest in people, especially the chronically ill. • Be flexible and adapt to changes readily. • Work well with various levels of personnel. EDUCATION: Home Health Aides are certified by the NYS Department of Health after successful completion of 75 hours of training, which includes classroom and supervised practical training. Candidates for the training program should meet or exceed the minimum criteria specified in the State's policies and standards for individuals rendering Home Health Care services. SPECIAL REQUIREMENTS: Must possess a current New York State driver’s license or State Issued ID. Availability of a motorized vehicle is preferred or access to other reliable means of transportation. Travel is required to branch offices and clients’ homes. PHYSICAL DEMANDS: 1. PHYSICAL ACTIVITY: Moderate physical activity, such as carry groceries, patient transfer, and light housekeeping, in accordance to Patient Care Plan. Noise and /or level of distractions in the work environment is moderate and should be expected. 2. WORKING CONDITIONS: Due to the nature of this job, you may be at risk of being exposed to potentially infectious blood and body fluids. It is to be understood that you will follow Universal Precautions and wear personal protective equipment as appropriate. Exposure to various living environments PERSONAL APPEARANCE: Expected to present a neat and well-groomed appearance in accordance with the VNHC Dress Code Policy. Photo identification badge is required to be worn at all times when outside the Agency.
